I do not know of anything built-into Jira to do this.

What problem are you trying to solve by performing such an action?

I am looking for something link a questionnaire allowing generating matrix of agile maturity (I already found some excel sheet).

My question was about questionnaire generating automatically (from the answers) the agile maturity matrix.
