Natively Jira doesn't have forms available presently

If you wish to enable forms in your jira software, request you to raise a ticket to https://support.atlassian.com/.

The team will first validate your instance and check if the feature of forms can be enabled from the backend.

Currently it is not possible to add a form in a Software project. Forms are currently only available in Work Management projects, but hopefully as part of the merge of Software and Work Management it will be available in the future.
