Would like to create a Disclaimer box in Service Management Project

Hi Team,

Our team have created a Jira Service management Project, in which we would like to have a Disclaimer box in Service Portal while creation of ticket. When a customer is ready to create a ticket, it should show up a Disclaimer box having some content in it. 

Then after creation of ticket, the Disclaimer box needs to disappear.

The problem here is, we can able to create a disclaimer, But it is showing up after creation of ticket.

Any help would be greatly appricated.
