Why "Create Issue in EPIC" is not working?

I created an EPIC. Then clicked on "Create Issue in EPIC". Chose the Issue Type, Filled the Title and then clicked Create. Post that an issue creation pop up came up. Filled that and clicked on Create, another issue got created and that didn't show under the EPIC. What am I doing worng?

@edwin vasquez I am also having same problem for one project but not facing same for another one. What could be the issue?

I just walked through the process you described and it works for me. Can you post a screenshot that of where you are clicking or when "Post that an issue creation pop up came up" ?
