I asked Rovo and it says I should not need any additional permissions, however, when I am trying to create my form, I want to create a form field that I can map to the Description field in my EPIC.
I am unable to find where I can begin this process, Rovo says it should be on the page, but I cannot find it.
Can someone help?

In what Type of Space are you creating the form? Is it a Business, Software, or Service space?
The process for creating forms for Business and Software spaces is different than for Service spaces.
For a Business or Software space find the Forms tab among the tabs at the top of your display: i.e.
In Business and Software projects you cannot create a new field in the form. You can only add to the Form fields that correspond to fields in your work items. So find the Description field in the list on the right, and drag it into your form.

It is company managed, and I am trying to add a Form-Only Field
Form-only fields are allowed only in Forms created in Service spaces. That is not allowed in Forms created in Software or Business spaces.
What is the Type of the space where you are creating this form?
You can get that by clicking on the ... button next to the Space name in the panel on the left and reviewing the last two lines in the pop-up.

go to:
Project settings / Space settings → Request management → Forms → edit the form
Mainly Form only Fields you creating Directly in Form Editor and then Dragged with Drag and Drop - @Trudy Claspill Covered explanation here and nothing to add.
Space Type is important, as Trudy stated, so Check what she asked - That will give you then full clarity.
