Unable to map test fields using Test Case Importer

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
I don't see a way to map each field to its corresponding field in the test case importer, instead, I get a combination of field names as can be seen below with a greyed-out Next button.
