In software projects, whichever fields are added to the Issue Create Screen will be available to add on the form as well.

You may review the create issue screen and check if the required fields are added or not.

PS. You may remove the URL or mask the details like your site name or the project details etc.

Another thing is you'll mostly find other Atlassian customers and partners, and not so many Atlassian Support members. That being said, we cannot access other instances to troubleshoot problems further.

If you're Jira admin on your instance, I'd probably ask you to share some additional details—like what's the requirement, what error message/behavior are you seeing... If you could share some screenshots, that would be helpful as well.

In case you're not a Jira admin, I would recommend reaching out internally to dedicated Atlassian support staff to see if they can sort things out. 👀
