Trying to Create a Custom Screen for Customer Issues

I am trying to create a custom screen for allowing our customers to report bugs, improvements and new feature requests. I don't want to use the default issue screen because it has to many fields that our customers wouldn't know or need to fill out. The only fields I am looking for are the:

Issue Type

Summary

Description

Reporter

Priority

However I still want to use the default issue screen for our internal issues that are open. I was able to successfully configure the Reporter field for optional and give the proper permissions to Anyone for Create Issues and Browse projects and this allows anonomous users to submit issues but it still requires too much fields for our customers. What am I doing wrong?
