In addition to Jira Software, what additional licensing is required to enable issue type Features and/or Initiatives within JIRA.
Also can you change the hierarchy and add features and Initiatives without a premium license?
Technically you can add the issue types for "Features" and "Initiatives" without the Premium subscription. But Jira would not recognize those as higher in the issue hierarchy than Epics without the Premium subscription or a third party app that adds portfolio management functionality to Jira.
If you create standard issues types (the same level as Story, Task, and Bug) and name them "Features" and "Initiatives" you could use generic issue linking to artificially link issue in a "parent/child" manner. But, as I said, Jira would not treat those as "parent/child" relationships. You would have to use custom automation or third party products to try to leverage the "parent/child" generic link in reporting.
Structure by Tempo is one of the add-ons you could use instead of upgrading to Jira Software Premium. We've used it successfully for several years. We found it to be more adaptable to our process than Advanced Roadmaps, and their support and development teams are very responsive. Check the website in my profile for more info on how we did it.

To use Initiatives and other issue types above the Epic level, you will need a Jira Software Premium subscription.

You can use issue links like "parent of" etc to create your own hierarchy.
However you would need a custom app to visualize this type of hierarchy based on Issue Links.
