When creating a form, there's a required field called "Summary." However, when accessing the form via a separate link to start filling it out, the "Summary" field does not appear.
Found a fix! Summary wasn't included in the Work Type Screen. Once Summary is added, the field will appear on the form.
Project Settings > Work Items > Types > select Issue Type that form is for > add Summary to list of fields

Summary is always a required field. Could you add a screen shot of how the form is configured so we might be able to help you troubleshoot how it's missing on the form?

I'm having the same problem, here is a screenshot of what the form looks like (Summary/Candidate Name doesn't appear), and the form builder which has Summary listed as required. The field doesn't appear anywhere else on the form.
