We have created a script which integrate salesforce and GitHub with JIRA.
While creation of issues we have an option for login in either GitHub or salesforce and then put the change request related to these plugins for performing any changes. after entering all the details, it creates issues in JIRA project with all relevant change information related to salesforce/GitHub.
Can we have similar kind of feature/add-on anything available in cloud JIRA software?

You could probably give ScriptRunner Connect a try as well to script the integration between Salesforce, GitHub, and JIRA.
They all have REST API, so it should be to chain the processes together if it is for/from Salesforce/github. There are some templates and managed connectors which helps as well.

This sounds quite custom - is it, or is this using some other external app / middleware?

The final option is to consider a middleware, whose licensing isn't linked to your user tier. Two of the Apps listed above are from Exalate, which is an example of a middleware.

There is a configuration page where they specify to link the other software currently, they linked Salesforce and Github.
They have few projects in which its configured while creation of some specific issues.
Like for example If someone create "change request" issue for making changes to a software, while creation they need to specify the new release which they wanted to integrate with "SPL Assests", Click on add "SPL assest or CI" option then we need to enter username and password.
Once credential verified it give furthermore selection like assets, branch, current, previous release etc.
