Product or Plug-in for Database Tracking

Does Atlassian have a product or plug-in that tracks database schema changes, and links database versions against the development tickets that include information about the reasoning behind the change (e.g. a customer ticket), the development work and testing of the change?

Does it track changes to the database schema, and records database schema versioning over time and keeps copies of database schema versions from any point-in-time?

In a release cycle, does the product produce reports of database changes, so that any internal and end-users relying on the database schema can update their queries accordingly?

I looked through Atlassian's tools, but I could not see any tools that filled this documentation role in the development cycle. I am not a developer though, so I may have missed a common tool that everyone else is using.

Just wanting to streamline our current process.

I found Redgate DLM Dashboard on the internet, but that software was discontinued. Probably looking for something a bit like this, but built to work in an Atlassian eco-system.
