'Parent' field not showing up in Form

I have created a form in Jira software. One of the fields in the form is the 'Parent' field to like a parent item - this is a required field.

When testing the form before, the form was working as expected and showing the 'parent' field when opening the form. Today I tested again and the 'parent' field is not showing up in the form through the form link. See screenshots attached.

Hi Ika - Welcome to the Atlassian Community!

It looks like you might have adjusted the standard work type hierarchy. Is that the case? Can you verify that the hierarchy has not changed? Can you share a screenshot of the hierarchy here? 

What work type is the form connected with? And what is the work type of the parent for the form work type?
