My members and I have noticed that Jira notifications are not always being delivered as expected. Sometimes they come through, but in other cases, no notification is received.
Could this be related to our notification scheme or project settings, to individual user notification preferences, or to limitations of the free Jira version that might prevent certain notifications from being sent?

Yes to all of those.
Regarding the limitations on 100 emails per day on the free plan, when that limit is hit all emails will stop, not just some emails.
Otherwise, emails sent depend on Notification settings for a project, Personal Settings for notifications, and can be affected if the recipient's email has been added to a suppression list by Atlassian.
