Need help in setup of Customer Satisfication Survey post closure of tickets.

We need to setup the satisfaction questionnaire for the customer for the EDI tickets. These tickets are raised by our team or Mc Kesson IT Team for our team.

 Post the ticket completion, we want to trigger the satisfaction survey E-mail to the end customer. So, I was found the below option for the Satisfaction Settings in Jira ( as shown in the screenshot

I have found the option in Jira - Project Setting - Satisfication Settings for the same and found that the Feedback is "ON" for my Jira project.

Now need your help in setting up the following.

  1. Trigger the Satisfaction Survey post completion of ticket to the Customer Email ID.
  2. Need to add 5 to 6 questions for the Survey. Need your help in how to add more questions.
  3. As Reporter for most of the tickets are Mck IT Team or Our team (tickets created by our project team member) need the change in the Email ID option after post completion of ticket , so that the Survey will go to McKesson Customer / End user as per our need.
  4. Change in the Format of the survey apart from the shown in below.

Hi Navin - Welcome to the Atlassian Community!

You cannot add questions to the built-in survey in Jira. It is intended to be very simple and basic. There are add-ons that you can find in the Atlassian Marketplace that will help you with the items you want. Apart from an add-on you will not be able to do those things.
