Migrating Standalone Jira to Jira Ondemand

I am currently running Jira 4.0 and am investigating the migration to Jira OnDemand. Before I go down this road, I had a few questions:

- Should I upgrade our Jira 4.0 to the latest Jira before migrating to Jira OnDemand?

- We have custom workflows and fields, can we still migrate it to Jira OnDemand?

- We currently tie our Jira 4.0 to our local LDAP environment. What will need to be done in order to decouple those components before migrating to Jira OnDemand?
