There is not a simple answer to those questions. Each is a large topic on its own.
I recommend that you start by reviewing the information available from Atlassian here:

You will have to review information from each plugin to determine if the vendor provides a Cloud version of the plugin, to determine how to migrate the information, and to determine if there is feature parity between the server version of the plugin and the cloud version of the plugin. You can generally find links to that information in the plugin's Atlassian Marketplace listing on their Support tab.
You mention Confluence and Bitbucket in your post title. Do you have Jira integrated with those products? Are those products also self-hosted? If so, do you intend to keep them self-hosted or do you plan to migrate the content of those products to the cloud also?
If you have a large environment, one with many customizations, or one where you use many plugins you may want to consider consulting with an Atlassian Solution Partner that specializes in Cloud Migrations. You can find such a partner in your geographic region through the Atlassian Partner Directory.
