JIRA service integration with Sharepoint

We have a Service request process using JIRA Service desk to create AD groups .However people sometimes enter wrong data or instead of creating multiple tickets they enter everything in one.

In order to get the correct data we are trying to send a SharePoint form URL within the JIRA service request form so Users can enter the data in SharePoint form. Is there a way we include the sharepoint URL and save the JIRA ticket number which will be created using the respective JIRA form?

FYI-: We don't have the JIRA -SharePoint connector.
