Issue with custom workflow

Hi, I have tried to customize the workflow by adding two more steps, UAT and Code Review under the "In Progress" status. After I did some modification my "Done" status got a button labelled as "To Do" and the "To Do" status got a button labelled as "To Do (2)"... I'm totally lost now and cannot fix this.

Earlier today, I've found the Transition name for "Done" was "To Do" and for "To Do" was "To Do (2)". I've corrected them. However, "To Do (2)" is now corrected to "To Do", but "Done" is still being displayed as "To Do".
