Issue gets created without mandatory field

User is able to create issue without filling mandatory fields, this is happening very rarely but surely it's red flag.

I am not sure how this happening.

What makes you think the field is mandatory? Where did you make the field mandatory and how? You need to check the Field Configuration Scheme for the project to make sure the field is marked as Required there. 

If there are multiple Field Configurations in the Scheme, then make sure it is required for each issue type/field configuration.

You'll need to look at the issues that are missing the fields to establish if there is a pattern, and check if there are automations, listeners or post functions that may be removing what the users have entered.
