Issue Collector > Full Name and Email fields disappear after editing

I created an Issue Collector for my project, and I was able to insert two additional fields into the form - Name and Email. 

However, I edited the fields (just so Name is 'Full Name') and in the issue collector form editor it looks fine, but in the form itself these fields disappeared.

Why is this happening and how do I get these fields back?
