Issue Collector Hidden Append Fields before Issue Creation

I'm trying to optimize an issue collector we have in our Atlassian wiki. Ideally I want to append the description before the issue gets created but not show the user. The idea is I've populated the description with a sort of form that I want the requester to fill out. When they hit submit, I want to be able to add a checklist that the requester won't see unless they look at the ticket in JIRA. Is it possible to append the description field in such a manner?

To clarify...

  • The Issue Collector has a form for users to fill out?
  • When they select submit, the Description should be modified with additional information - in this instance, a checklist?
  • Is this a checklist from an App? Or just a list? 
  • Is this checklist dynamic or static?

It sounds like this could be possible using Automation - but I think it'd be good understand more about the process, how the Issue Collector looks, and how it should look in Jira with the list.

If you've got some screenshots, that would be ideal.

  • The issue collector doesn't have a form. I "create" a form by populating the description field using javaScript. When a user clicks a link in our wiki that is connected to the issue collector, the description looks something like the below example:

  • Correct. I want to append to the above list of questions (form) in the description field after the user hits "submit". This would mean the requester doesn't see the list. An example of said list is below:

  • Just a list, text that I define and unchanging
  • Static, unchanging

It might be possible with an Automation App - do you have one? Such as Automation for Jira, or Scriptrunner?

Although I think it would be easier to either...

  • Place the list in a custom field, and set it using default field values
  • Use a checklist App from the Marketplace - this would be a more user-friendly option also
    • Depending on the App, you might still need an Automation App to set the checklist at creation
