Is there a way to create a bug template

When I am submitting a bug/issue, there are certain things that are common such as version, DB version, Setup, precondition, Steps to reproduce, etc.

I would like to have a template with such a list

Your best option would be to create each of your common items as fields, add the fields to a screen, then make it so that screen is shown to the user when creating bugs.
