Is it possible to have customized request form in scrum project ?

Hi All ,

Can any one help me ? 

Please check the attached screen shot. For one of the scrum project , project team wants to have a customized request form like this. Is it possible to have ? if it is possible can this be done by project admin or does this required jira admin help ?

Hi, 

You can have all fields that are in you screen but you can't have a create screen or edit screen similar to yours. Jira doesn't allow you to choose the fields position like your screen. Every field is positionned in one column.
