Is it feasible to add custom field in column format

Below is the requirement, for one of the intake form user wants to add Following fields; 

Lead Consultant;  Capacity Percentage; Expected due date

Support Consultant 1; Capacity Percentage; Expected due date

Support Consultant 2; Capacity Percentage; Expected due date

Support Consultant 3; Capacity Percentage; Expected due date

By column, do you mean to see the custom fields in columns like this? one field next to the other?

Capacity Percentage;

Expected due date

And like a drop-down menu to select the values?

Sadly not with Jira software. Jira JSM -> forms you can.
