I need help locating the app add-on configuration page

I am attempting to set up the Jira Server App for Microsoft Teams (https://www.msteams-atlassian.com/jiraserver/index.html#jira-id) and am having trouble following the directions. I cant seem to locate the Jira ID or Public key.

Any help is appreciated!

I can't find this either. No search 'on Jira Id' in the search box was helpful either.
