How do I write a custom report by Epic/Assignee within Epic for a specific time period which will show me the amount of time spent (per ticket) which I can then export to excel for subtotals and totals?
We don't have Confluence.
(Need step by step because I'm not familiar with writing custom reports out of Jira.)

You can use Jira Rest API's to pull this data and build on your own custom solution. Or if you would be interested in a mktplace app , take a look at
The app shows your Epic hierarchy in a tree view and time spent rolled up at each parent level. Also we have detailed time spent report as show in the pop up below which can be exported easily to a csv file.
The app has multiple other features which are very useful in project tracking and management.

If you are open to using the apps available on the Atlassian marketplace then I can recommend the Worklogs - Time Tracking and Time Reports app which was created by the company I'm representing.
The app will allow you to create your desired report and then export it as Excel file. In the example below I generated a report which grouped time logs in the following order:

There is not functionality available natively in Jira Cloud to create that sort of complex reporting of time spent. That will require a third party app.
