i'd like to see if there is a method of logging incidents on an issue and make a report of all incidents per issue.
is this possible?
How are you telling Jira that there is an incident against the issue?
Comments? Sub tasks? Linked issues?

an incident is in this case not necessarily an action, it would be discussed and then decided if it becomesan action.

Sub-tasks are a good way to do it, because everything is in one place. Even if a particular sub-task leads to no action, you can have a workflow that records that easily.

The issue will display all the sub-tasks, but I assume "report" means something other than "look at the issue".
I can't list possibilities, there's far too many with lots of explanations potentially needed. It would be easier if you defined what you wanted in your reports, then we can guide you towards something to do it.

With that level of detail, this is probably little more than "search for issues and select the columns you need to see in the issue navigator". Rather than sending this, I'd encourage the PM to use Jira directly for themselves, so tehy can focus on what they need, rather than relying on you to hopefully include it.
