How to have Description Field Example Preview Look Like This?

I love how the Description field in the Request Form looks like this. While editing my project's request forms, I cannot figure out how to get the field to look like this. PLEASE HELP!

Welcome to Atlassian Community! 

You can only example text to the description field if you are using a team-managed project. This feature is not available in company-managed projects.

I believe it is a team-managed project, but how would I go about confirming that? I am an admin for the project.

If you look on the left-hand section while in the project, at the bottom it will tell you what project type it is.

Okay, so that means that your project most likely is a company-managed project. There are a handful of JSM projects that can be created as team-managed geared towards Customer service, HR, Facilities, and Legal service management.
