How to create a customized inquiry form?

Good morning,

I would like to create a new form of inquiry for business travels (to be filled in by requestor) with a form alike that attached in the screenshot. How do I add customary fields in this inquiry? Could you describe the way I can do it a little bit more comprehensively than in documents (https://confluence.atlassian.com/jiracoreserver071/process-management-with-jira-802172782.html)

Hi Victor - Welcome to the Atlassian Community!

You can create custom fields by going to Settings > Issues > Custom Fields. 

After you create the new field, you can add it to one of your existing screens.

Actually, I have already created the required fields, but I can't get how I add them to a screen.

Go to Settings > Issues > Screens

Find the screen you want to add it to and click on Configure.

Or you can go to Settings > Issues > Custom Fields

Search for the new field, then click the 3 dots menu and select associate to screens.
