How to create a Dropdown Custom Field and on selection add checkbox custom field

i am creating a dropdown custom field and based on selection from dropdown i am expecting a checkbox of selected content .Thereafter, on button click the checkbox content should be added in textfield. Eventually ,when i go to create Issue Request i will be able to select multiple options and send request for multiple things.
