How to configure Questionnaire custom field

We would like to configure a Questionnaire in JIRA in the detailed view of an issue

For example

If a question is answered " Yes " there will be list of questions that will be displayed.

If answered "No" no further questions will be displayed.

Is there any custom field or free plugin in JIRA for this feature?

You can do this using java script. You can show/hide the custom fields on the screen using script.
