When the value changed in Actual effort or Total effort for defects field, then the value should be added and updated with the new value i.e. (Refetched) and showed as Total actual effort.

Please show us the entire rule.
You said the fields are "plugin" fields. Do you mean they are fields added by a third party app? Or are they custom fields using a field type provided by a third party app? What are the actual field types of these fields?
In your formula you reference two custom fields by ID. What are the names of those custom fields? How do you know those are the correct field IDs?
Is the rule executing? If so, please show the details of the Audit Log for the rule execution? and tell us what result you are getting.

Yes, 'Total Efforts for Defects' custom field was developed by third party app because of that it's not reflecting in reports and issue level!
And its executing when we open the other task but not automatically.

What other app are you using that created the Total Efforts for Defects? Did you app create the field, or did it provide functionality that allowed you to create the field? Can you provide the details on that field's configuration? Is the field listed in the Custom Fields page? If so, what does it show there for the field Type?
You said the Total Efforts for Defects field is not reflecting in reports and issue level. If it isn't visible there, then where are you able to use it?
What is the field type of the Actual Effort (Hrs) field?
