If you are asking how you require the reporter to specify a fix version at the Create do the following. Commentary- it strikes me odd that the reporter would specify the fix version at all much less at create but...
- create a screen and include the Fix version field
- edit the workflow and edit the Create transition
- add the screen to the transition
- add a Validator (field required) for the Fix version field
- publish the workflow
- test
NOTE: if your Bug Issuetype is sharing a workflow with other issuetypes you will want to first create a unique workflow for bugs...
- copy the workflow
- edit as above
- associate the new workflow to the Bug type
