I am setting up an Active sprints board and would like to see the Epic associated with each ticket like this:
I am currently only able to see it like this without the Epic listed:
How do I get my sprint board view to include the epic?

Is there any Epic associated for this second issue GPA-152 as you have for GPA-139? The issues it seems are of the same project, so if they are at the same board, the configuration is the same for both.
Confirm if there's an Epic associated for your second Issue. The Epic needs to be on the 'Epic Link' field of your task.
The first screenshot is from an old board that someone sent me the screenshot of to hopefully mimic.
The second issue does have an Epic associated with it and is linked in the Epic Link field

Welcome to the Atlassian community.
Are you working with a Team Managed project or a Company Managed project? That information will show at the bottom of the panel on the left when you are viewing the board.
Do you have Swimlanes configured on your board? If so, to what are they set?
The parent Epic of an issue should display in the card automatically unless you have the Swimlanes set to Epic.

It's a company managed project and I have the Swimlanes already set to Epic.

When I tested this yesterday I swear that when I had the Swimlanes set to Epics then the Epic data will not automatically display in the card because that is displayed in the header of the swimlane.
However when I do tested it again just now the Epic info did still display in the card, on a Company Managed Scrum board.

Can you show us a fuller screen image from your board?
The Swimlane setting is per board. There is not a system-wide option to disable it for all boards.
