How do I personalize default screen for new leads in a Lead Tracking project?

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!

I have started a lead generation project where Leads are created with prospect information. 

Rather then having team members listing out:

  • Customer Name:
  • Phone number:
  • Company size: 
  • etc.

I would like the system to automatically populate fields with the needed items as a default. I am not sure if these can be done and if so if it is considered a Issue Type Screen Scheme or Field configuration scheme.
