How do I guide the user to relevant fields based on the response in a Form?

I need the user to see only the relevant questions based on the previous response. How do i set this up in a form?

If it is only one level deep, you might try using a select list cascading type of custom field.

Or you can also use the ProForma Forms add-on.

Use Form logic for such a case by Smart Jira Forms add-on. When/then logic helps to create dynamic forms based on the previous response.
