How do I add a custom field that is automatically populated on the Create screen?

Hi there - we have Jira Software. I have found forums on Jira Server that allowed this custom field, but do not see how to accomplish this in Jira Software. 

When a user clicks CREATE at the top of the page, the create screen opens up and I want an information box next to space to educate users on what space to select and another information box next to work type that explains to the user what work type to select. 

End goal is we want users to complete intake direct to Jira so if someone has a better idea, please let me know. 

I have seen project forms, but that seems like it will get cluttered as each space will require a form.

Hi Natalie - Welcome to the Atlassian Community!

When you create the custom field, you can add a Description for the field that should be visible to the user on the Create screen. This is where you would put in information to guide the user.
