I’m working on a Jira-to-Jira synchronization use case where issues are synced between two different Jira instances (Cloud → Cloud). I’m looking for best practices and recommendations on handling rich text fields (ADF) along with attachments during the sync.
Sync issues between two Jira instances
Support both:
System fields (Description, Comment, Environment)
Custom fields of type Rich Text
Handle issue creation and updates
Suggest to me the best approach to handle the RichText fields with attachments?

If you want this to work across Jira Cloud instances, Exalate is built exactly for that, ADF‑aware rich text sync plus attachment handling out of the box, with room for custom rules when needed.
It can keep ADF end‑to‑end for Description/Comments/Environment + rich‑text custom fields and sync attachments separately to then update the ADF that references them so inline images stay intact.

as @Marc -Devoteam- and @Clara Belin-Brosseau_Elements already mentioned that's the classic use case for an issue sync app. Some people tried that with Jira Automation but what's still not possible afaik is the handling of attachments.
My link above leads you to the marketplace where you can find a list of all the issue syncing apps - all of them should be able to fulfill the need which you've described above. They differ a bit in the way they work: Setting it up via scripts, UI - how you set things up.

You can synchronize issues across Cloud projects very easily with all their associate fields.

I think your best solution is a 3rd party app.
As you can only generate a download link to get an attachment via API.
Attachments has always been an issue with in Atlassian.
You could look at Elements Copy & Sync or Deep Clone
