We are looking to create simple, customizable forms in Jira Software (Data Center). Specifically, we’re interested in using dynamic forms in tasks and mapping fields after submission.
Is there any plugins for this?

Jira Software (Data Center) doesn't offer native form functionality like Jira Service Management does, or Software projects on Cloud, but there are several plugins that fill this gap. One solution that fits these needs well is Smart Forms for Jira (Data Center) developed by my teams.
Smart Forms for Jira can:
- Build dynamic forms and get a wide array of form elements and content options, far beyond basic Jira fields, without the need of creating custom fields to make your forms clear and user-friendly.
- Use conditional logic to show/hide fields based on user input
- Map form responses to issue fields (e.g., summary, priority, custom fields). Once the form is submitted, those Jira fields are automatically updated.
- Reuse forms across multiple projects or issue types
- Track and review submissions in one place, withing responses tab.
