We're looking to incorporate a pass/fail rate for our sprints in terms of whether or not initial sprint commitments were met.
Today we're tracking this in a spreadsheet and requires asking each PO about the sprint status at the end.
Ideally when the teams go to complete a sprint in JIRA, I'm looking to have them prompted with a yes/no question on whether their team passed/failed the sprint and pull that into a report.
I don't want to have this automated by JIRA so that it automatically pulls whether or not all of the the tickets were completed, because our teams may complete all of the tickets they estimated, then pull in new ones. The new ones aren't always completed, but the criteria we want to rate on is "did you finish what you estimated at the beginning of your sprint" not "are all of the tickets currently in your sprint done".
Does anyone know if you can add a question when completing a sprint? And if you can pull reporting off of that?

Unfortunately, we are not aware of any plugin that would pop-up a question when closing a Sprint, however, I think you can use a different option to achieve this same purpose:
Create a single Master Epic for each Sprint you start. On this Epic, you will add the general questions you want and use the inbuilt reports of JIRA to track on which Sprints this Epic was completed or not.
Additionally, you can link this Epic as a parent of all your Sprint issues, so your project teams will not forget about it.
