As an Org Admin ..... why can't I update this user's profile avatar / pic ? Why it is showing as 'External user' and 'details managed by user' ?

The account doesn't "belong to you" even though you are the Org Admin for the space that might have done the original invite. The account is an Atlassian account and belongs to the owner of the account, which is the user in this case. If you implement Managed Users, then you can control a few things like the name and the email address but not the avatar. Only the user can update that.
@Michael Green in cloud there is not a way to change this. The E is just the default since they have not chosen an avatar.
