i have created a request form for users to complete when sending information to us however i need to add a yes/no checkbox onto the form through the visible fields...
can you assist?

go to admin -> custom fields, and select checkbox type, provide the field name and yes/no values and create it.

i dont have custom fields in my section?
i currently go to admin -> Request Tyes -> choose the request form -> add Field -> from here i only have a limited number to select and no custom field available?

First you need to create this check box field in jira using admin menu and add the respective field to create issue screen on that respective issue type.
Once you done this then only this check box field will be available into Request Tyes -> choose the request form -> add Field page.
