Add values to a drop down menu in a form

We want to create a form and fill the options selection of a drop down menu with the values from a csv file. Is this possible and if so, how? Many thanks for the help.

No, Forms does not except data from a csv file, currently it only support JSON information via a data connection. There is a suggestion to add support for Google Sheets and csv, see JSDCLOUD-10852. I would recommend that you go and vote and follow that suggestion to see if Atlassian decides to add that feature.
