Based on our experience as Raley Purchase Orders app vendor, purchasing management is far more complex than simply submitting a form to create a Jira issue.
As you rightly pointed out, normally you need a several items per purchase request. If you solve them via subtasks where each subtask represents an individual line-item, this only adds complexity in calculating the totals and approvals routing. Not to mention the fact that the purchase request should be logically addressed as one task, not several tasks.
Copying a table with all the line items and putting them under a single Jira task also has problems as you need to calculate taxes, totals, budget associations etc. It becomes even more complicated when you need to run an approval flow which depends on the Gross total, budgets referenced and possibly new products/services approvals.
Even worse, at some point, you'll need to have the figures - what is budget X and how much of that is committed, spent, paid etc. And there're really many budgets.
Having said that, in purchasing management even a super-smart form is just a beginning. To run the process well on Jira/JSM you'll need an app or implement an in-house solution.
