trasition in the support portal: Is it possible to customize the fields in the dialogue?

Hello all, 
our team is trying to transit a work item from the portal.
Tha is working but we would like to enricht the screen somehow to add some instructions and / or some logic.
Is this possible somehow?
Until now the only thing possible is add a simple comment :(

Out of the box you can't do that unfortunately. You can achieve that via a 3rd party app called "Actions for Jira Service Management" https://marketplace.atlassian.com/apps/1213697/actions-for-jira-service-management?hosting=cloud&tab=overview
