Hello, 

I would like to know if it's possible to create a dynamic form on a single type of request

I've try to create two forms and to link them both to the same request but i can just see one.

If you know how i can choose between the two when the user is on the request .

Please can you share what setup you did to achieve this, I was try to add 2 forms to the same request type, but didn't work at my end.

(I can add another form via the Agent View, but that isn't my end goal)

you really should just use one form per request type. You should have different request types based on the users need.
