Wizard to guide a user to the corret form to submit

Hoping someone can help with the following scenario for a user

Give the user a list of questions and then provide them with a link to the correct form they need to complete for the particular issue. i.e the reporter answers a number of questions and based on the answer they need to complete an IT issue form or another set of answers means they need to submit a software bug form.

@Damian Martin There is nothing like this in the delivered tool and I am not aware of any app that would provide this.  You could use forms to create a cascading questionnaire and then complete fields in a request based on what they answer in the form.  The issue here is you would not be able to have multiple request types.  It would probably be best practice to have good descriptions of your request types and maybe a knowledge base article that users could use to help direct them to the correct form.
