Currently I am trying to learn to create forms for our service desk. However, when I go to create a form, there are no templates like the instructions on https://support.atlassian.com/jira-service-management-cloud/docs/create-or-edit-a-form/. Can anyone offer any advice. I am still relatively new to Jira and am not an expert by far.

Unfortunately, it seems you are running into a known behavior that is being tracked in the following bug report:
We encourage you to add yourself as a watcher of the bug ticket to follow up on its progress, hit the "This affects my team" button and provide any feedback you might have on the comment section (make sure you're logged into the system). This helps us in understanding the impact that it might be causing to you and other customers. On top of that, it creates a path of communication between all impacted customers with our developers and product managers.
I know that isn't the news you'd like to get and our apologies for any inconvenience this might be causing to you and your company.
As this has been reported to our developers, I've marked this ticket as "Resolution suggested" in favor of watching the bug ticket for an update.
I am experiencing the same problem. I am a Global administrator and I don't see the option to select templates. I also don't have the ability to add forms to an issue.

@Joseph Chung Yin Yes. I am my companies Jira Administrator. I am setup in the project administrator role in all of our projects.

This is very odd because you have all the proper permissions based on your response. You should be able to access all the templates when creating a form.
I would recommend you to submit a formal support request to Atlassian Support Team for further assistance.
