Why are our feedback widgets sending tickets to the wrong projects?

We have two software applications with embedded JSM feedback widgets. The widget on Platform A is routing feedback to Project B, but we want it to go to Project A. How can we fix this issue? 

Another thing that might be relevant: When I go to Project Settings --> Widget, there is an error that says, "The widget is turned off because there’s a problem with the default request type. To fix the widget, choose a new default request type. If I select a Default Request Type, the widgets temporarily work and route feedback to the right locations, but if I return to Project Settings --> Widget, the same error about issues with Default Request Types shows up and removes the selection.
